Please may we request that all practices continue to refer all patients requiring home oxygen to the home oxygen service using the emailed referral form. As a reminder, they operate 7 days a week from 8am to 6.30pm on weekdays and 8am to 4pm at weekends. All referrals marked urgent will be strictly prioritised. It is a direct access service and does not require referral to IHT or WSH respiratory departments which will incur an OPD charge.
Please note that the only referral exclusion is paediatric use. Children who are receiving supplementary oxygen should be under the care of a paediatrician. The Service is able to provide advice on cluster headache patients and for patients at the end of life.

Patients will have access to electronic re-ordering and holiday request forms as well as the usual telephone services – BOC and the COPD Service will advise patients.
As a complete fail-safe, should there be any circumstances outside of the referral process in which a GP still needs to order oxygen, the new Part A HOOF and accompanying IHORM forms can be found on the right-hand side. These should be returned to firstname.lastname@example.org, or completed via the BOC Home Oxygen Portal.
If you have any concerns about the safety of a patient using oxygen, which may relate to fire risk or clutter, please refer them to the Suffolk Fire and Rescue Service online form to request a Safer Home Visit where eligible. There are also further leaflets relating to fire safety available on the Suffolk Fire and Rescue service website.
The CCG has also produced an information form on oxygen safety for use in Primary Care for patients and their families.
