Serious incidents requiring investigation (SIRI)

On identification of an actual or potential serious incident, we request that an initial report form giving the basic information on the incident is completed and sent to the Chief Nursing Officer.

Examples of SIRIs in general practice may be:

  • Serious incidents involving patients e.g. screening errors, serious drug errors
  • Serious injury resulting in permanent harm, or unexpected death involving patients, a member of staff, visitor, or a contractor
  • Not following guidance on the ‘yellow sheet’ (available to all practices) when there have been serious concerns on a child’s welfare
  • Any loss of data or breach of confidentiality

Information is sent back confirming the SIRI status and giving the date by when the 60 day report needs to be received. The 60 day is a full report with chronology and action plan.

The Chief Nursing Officer checks all reports for sufficient information, evidence of appropriate actions taken and evidence of shared learning planned or undertaken.
