You will all be aware of the significant pressures that we currently have in the Suffolk health care system.

The A&E department at Ipswich Hospital are encountering significant difficulties with a significant number of patients not being able to access services. 
Ipswich Hospital is facing huge demands, with a very high number of both walk-in and ambulance arrivals.

Frailty Assessment Base (FAB) 

Mon - Fri 9am - 5pm 

What is it? A six-month pilot based at Ipswich Hospital providing a consultant-led multidisciplinary service providing comprehensive geriatric assessment for fail older patients at risk of acute admission. All patients will be seen within 2 working days or sooner based upon clinical need.

Crisis Action Team (CAT) 

24/7 

What is it? A multi-agency scheme to reduce emergency admissions by supporting adults experiencing a "crisis" situation to remain in their own homes and/or enable rapid discharge from A&E to prevent an admission. 

How do I make a referral? Via the Care Co-ordination Centre on 0300 123 2425 and request the Crisis Action Team. 

PLEASE NOTE: Patients must be seen by a health professional within the previous 24 hours prior to a referral to the CAT team.
