An event is being held to explain the work of Connect Suffolk and how it is enabling closer working between health and social care organisations for the benefit of the local community.
People will have the opportunity to learn what help is available to support them and their family to lead healthy and active lives.
The event will take place on Friday November 16 at The Grove Medical Centre in Felixstowe from 2.30 – 4.00 p.m. and is being organised by the centre’s Patient Participation Group.
Emma Downes, Senior Social Worker, will speak about how the project operates and how the work it does is successfully supporting people’s physical and mental wellbeing
Connect was first established in 2015, with two trial projects in east Suffolk and Sudbury. It brings together health and social care services, county and district councils, police, church, the voluntary sector and community organisations. Connect aims to make sure people can access help and advice early on, so that they have support to look after themselves.
There are now 13 Connect Suffolk projects happening across east and west Suffolk.
Leisha Klein, chair of the Grove Medical Centre’s PPG, said: “Connect Suffolk is a genuinely new and different approach to helping people to live long, happy and independent lives.
“As a PPG, one of our aims is to support Felixstowe residents to live well. We are so pleased to facilitate this event and give our local population a greater understanding of just how much help and support is available.”
Paul Little, Suffolk County Council's Area Director of Adult and Community Services, Ipswich and East Suffolk, said: "Connect is all about services working around the needs of people and the local areas we serve and not the organisations we work for. People have told us that they want seamless health and care services, which reduce duplication and look at the whole person rather than just one symptom or one bit of support they need. We are working very hard to make that happen and will keep working to make it as good as we can."
